Student Registrations

Welcome to Hawkhill Medical Centre, we are pleased that you have chosen to register with our Practice and wish you all the best in your studies.

All students are advised to register with a doctor during their time in Dundee.  If you are here for less than three months, you can register as a temporary patient if you require medical assistance.

We require you to complete the TWO electronic forms at the bottom of this page to register with the Practice.  We cannot complete your registration until we receive both forms. 

Please do not complete these forms until you are physically residing in Dundee.  You may be deducted from your current GP list too early.

Application to Register with a General Medical Practitioner Form - please ensure that you complete all fields including:

      • "Home Address" box is for your Dundee address.
      • "Address in UK when last registered with a GP" box is your address back home.
      • "Name & address of previous GP Practice" box allows us to obtain your medical records.

New Patient Health Questionnaire form - this provides useful information until your medical records are obtained from your previous Practice.

The Practice may also contact you via email/text message for appointment reminders. Please tick the "Contacting you" box at the end of the form to consent.

We provide a service called "Patient Services" which will allow you to order your repeat medication online.  Please be advised when you tick the "Contacting you" box on the New Patient Health questionnaire form you will automatically receive an email from us to create an account.  Please follow the instructions on the email by going to
