New Patient Registration

Patients wishing to register with Hawkhill Medical Centre and who live within the Practice boundary, will require to complete a new patient registration form.

Practice Boundary

It would be helpful if you are able to provide two forms of identification when registering, one of which should be photographic (e.g. passport or driving licence) and one providing proof of your current address (e.g. utility bill, landlord lease).  New patients will also be asked to complete a new patient questionnaire form which will assist the doctor to treat you until your medical records are received from your previous Practice.
Patients are registered with the Practice rather than a named doctor but retain the right to see a doctor of their choice.

New patients who require repeat medication will have to make a GP appointment before a prescription will be issued.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes.  All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.
