CORONAVIRUS - COVID 19 UPDATE
Due to the COVID 19 pandemic the practice continues to be operational but the doors will be locked.
Please DO NOT attend the practice unless you have been advised by a clinician.

We hope you will appreciate these measures are paramount to ensure the safety of our patients and staff.
We understand you will be concerned and our practice team are working hard to ensure the continuity of our service. Please be kind and patient with our practice team.
Current Scottish Government guidance on the Coronavirus states that if you have a cough or fever you should self-isolate for 14 days from the onset of symptoms. This applies to symptoms of recent onset, meaning within the last 7 days. Members of a household where someone has suspected symptoms should stay at home for 14 days.
DO NOT COME TO THE PRACTICE
You can return to work after 7 days if you are improving but do not need to be symptom free. You should telephone the Practice if you are not improving after 7 days.

We are unable to test for Coronavirus at the Practice.
FREQUENTLY ASKED QUESTIONS
If you require a GP appointment please phone the practice at 8:00am as normal. The admin team will ask you questions regarding your symptoms and add your details to our triage system. You will receive a call back from a clinician who will assess your symptoms by telephone. If necessary they will arrange for you to attend the practice at an agreed time. There will be no advance appointments available at this time. All non urgent enquiries will be temporarily suspended.
If you are house bound and request a house call please phone the practice as early as possible in the morning and our admin team will add you to our triage system for an initial call back.
All appointments with our nurse and health care assistant will be triaged and if non urgent temporarily suspended until further notice.
All chronic disease annual review clinic appointments will be temporarily suspended. Please phone the practice if you require to speak to a clinician regarding new symptoms.
All family planning, minor surgery and joint injection clinics will be temporarily suspended.
If you have an appointment booked in the future these have been cancelled - if you require to see the GP, please contact the Practice that day and you will be placed on our Triage Call back list to be assessed.

-If you normally order through your local pharmacy please continue to do so. Please sign up to your local pharmacy and they can order for you.
-If you normally attend the practice please post your request through our letter box.
-Please phone the practice to register for our online patient service.
Please allow sufficient time for your prescription to be safely processed and signed.
We do not offer an on the day prescription service.
-If your prescription normally goes direct to your local pharmacy this will continue as normal.
-If you normally collect your prescription from the practice this facility will no longer be available. Please phone the practice and we will arrange for your prescription to go to your local pharmacy - please allow sufficient time for this to be sent to the pharmacy.
- The Pharmacies are under extreme pressure - please allow approximately 5 working days for them to dispense your medication.
We will only accept samples if requested by a clinician.
If you have been asked by a clinician to hand in a sample please phone the practice on arrival at the front door and we will arrange for someone to collect and complete our sample request form.
Fit Note and Letters
Fit Notes for on-going conditions will be processed in the normal manner, we will currently post these to yourself rather than collecting from the Practice.
If you are self isolating for 14 days as per advice from the Scottish Government on 16.03.20. A Fit Note is not required, however see attached standard letter Covid-19 Letter which may useful for your employer.
If you are new to the area and you would like to register as a new patient please phone the practice and we will complete the required forms over the phone. You must stay within our practice boundary.
Once normal service resumes you will be required to attend the practice to provide 2 forms of ID, one photographic & one with proof of your address.
If you require an appointment please follow the appointment process above.
If you require to register as a temporary patient, i.e. in Dundee for less than 3 months, please phone the practice and we will complete the required forms over the phone. Your temporary address must be within our practice boundary.

ONLINE PRESCRIPTION ORDERING SERVICE
The current email and website online prescription ordering systems is no longer available for you to order your medication.
The Practice is now operating a new service called "My Patient Services" where you can order your repeat medication more efficiently and also book appointments.
Please ask the Receptionist for a form to register for this service or alternatively click on this link to download the registration form: My Patient Services Registration Form
You will require to complete and return the form to the Practice before you can start using this service. Once we have received your form, we will send you by email your registration details which will guide you to creating your username and password.

Patient Assessment and Liaison Mental Health Service (PALMS)
Are you worried about your mental health? Feeling overwhelmed or low and needing to talk to someone about seeking relevant support?
You can self refer to PALMS for consultation.
PALMS is a new service run by Dundee Health and Social Care Partnership within NHS Tayside now available in your GP practice.

OPEN PUBLIC HOLIDAYS
The surgery will be open on the following public holidays from 08:00 - 15:30. The appointments will be strictly bookable on the day only.

Same Day Prescription Requests
Please allow sufficient time for the Practice Team to process your request. By law all prescriptions must be reviewed and appropriately prescribed. We cannot, therefore, guarantee that your request will be processed the same working day.
Please make every effort to request your prescription(s) at least 48-hours before they are required.
Our administration staff, and the Hawkhill website, can provide details about the ways in which prescriptions may be requested.
