Hawkhill Medical Centre

The Partners and Staff would like to welcome you to Hawkhill Medical Centre.  The present Practice was established in 1993 when the GPs from three well established Practices in the West End of Dundee joined together to form a new partnership to consult from a modern purpose built medical centre.

Hawkhill Medical Centre was designed with their patients in mind with a vision to provide a wide range of healthcare services

We are always striving to improve our knowledge and the quality of our services and we proudly achieved Practice Accreditation status and are also an Approved GP Training Practice.  Our aim is to provide all patients who are seen and treated by the Doctors and Staff at Hawkhill with the highest standard of medical care and efficient administrative services from our hardworking Administration Team behind the scenes.

CORONAVIRUS

You may be aware of the developing outbreak of a new virus strain in China, centred around Whutan City.  This virus can cause symptoms similar to a cold, or flu.  In some cases, it may cause serious pneumonia.

If you feel unwell, with one or more of the following symptoms:

-cough, new shortness of breath, fever - AND - you have travelled abroad (particulary to China or the Far East) in the last 14 days, YOU MUST stay at home and mimimise contact with others.  Phone the Practice (in hours, 01382 669589) or NHS24 (out-of-hours, 111)

THE PRACTICE OPERATES A ZERO TOLERANCE POLICY

The Practice considers aggressive behaviour such as personal, abusive and aggressive comments, cursing and/or swearing, physical contact and aggressive gestures.  The Practice reserves the right to remove any patient from our list who is in any way aggressive to the Doctors or any member of the Practice Team.

No abuse of Staff is acceptable whether verbal or physical.  All abuse will be reported to the Practice Manager/Office Manager and a log of all incidents will be kept.

All incidents of verbal abuse will be considered by the Partners and appropriate action taken, which may include the removal of the patient from the Practice list.

Any physical abuse of our Staff by patients or general public will be reported to the Police.  The patient will then be removed from our Practice list.  In these circumstances the patient will have seven days to find a new Practice.  If you are unable to find another Practice, Practitioner Services will allocate you one.

COMMUNITY CARE & TREATMENT SERVICES

Dundee Community Care and Treatment Service is a pilot scheme which was initiated by NHS Tayside with the Dundee Health and Social Care Partnership in February 2019.  This new service has been developed to support your GP practice.  The pilot involves all patients within agreed localities of Dundee.  Hawkhill Medical Centre is one of six practices included in the second phase of the pilot scheme. 

All patients over the age of 16 who require wound care, suture removal and blood tests will be seen by the Community Care and Treatment Service at another centre and will be asked by the practice to telephone the above service for an appointment on 01382 496717 (between 09:00 - 17:00, Monday - Friday).  The service will strive to give appointments that meet the needs of our patients and will work in partnership to ensure patients needs are fully met.

EAR SYRINGING (IRRIGATION)

Current medical evidence shows that ear syringing(irrigation) can lead to ear infections, perforated ear drums and/or tinnitus (persistent noise).  For this reason, Hawkhill Medical Centre will cease to provide ear syringing service from 28.08.19.  Patients who require ear syringing before attending an Audiology appointment will be referred internally by the Audiology Department for micro-suctioning.

NEW ONLINE PRESCRIPTION ORDERING SERVICE

The current email and website online prescription ordering systems is no longer  available for you to order your medication.

The Practice is now operating a new service called "My Patient Services" where you can order your repeat medication more efficiently and also book appointments.

Please ask the Receptionist for a form to register for this service or alternatively click on this link to download the registration form:  My Patient Services Registration Form

You will require to complete and return the form to the Practice before you can start using this service.  Once we have received your form, we will send you by email your registration details which will guide you to creating your username and password.

You can then order your prescriptions once you have created your username and password at:  www.patient-services.co.uk

"If you wish to register any other family members - you will need to fill in a separate form for each person".

Travel Advice Clinics

Please be advised that we no longer offer Travel Advice Clinics.  We are still able to offer appointments for administering NHS vaccinations.  For further information please see "Clinics & Services" tab at top of screen.

Patient Assessment and Liaison Mental Health Service (PALMS)

Are you worried about your mental health?  Feeling overwhelmed or low and needing to talk to someone about seeking relevant support?

You can self refer to PALMS for consultation.

PALMS is a new service run by Dundee Health and Social Care Partnership within NHS Tayside now available in your GP practice.

Same Day Prescription Requests

Please allow sufficient time for the Practice Team to process your request.  By law all prescriptions must be reviewed and appropriately prescribed.  We cannot, therefore, guarantee that your request will be processed the same working day.

Please make every effort to request your prescription(s) at least 48-hours before they are required.

Our administration staff, and the Hawkhill website, can provide details about the ways in which prescriptions may be requested.
