New Patient Registration
Patients wishing to register with Hawkhill Medical Centre and who live within the Practice boundary, will require to complete a new patient registration form.
Two forms of identification are required, one of which should be photographic (e.g. passport or driving licence) and one providing proof of your current address (e.g. utility bill, landlord lease). New patients will also be asked to complete a routine medical health form which will assist the doctor to treat you until your medical records are received from your previous Practice.
Patients are registered with the Practice rather than a named doctor but retain the right to see a doctor of their choice.
Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.
Guide to GP Services
The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide below.
