Hawkhill Medical Centre
The Partners and Staff would like to welcome you to Hawkhill Medical Centre. The present Practice was established in 1993 when the GPs from three well established Practices in the West End of Dundee joined together to form a new partnership to consult from a modern purpose built medical centre.
Hawkhill Medical Centre was designed with their patients in mind with a vision to provide a wide range of healthcare services
We are always striving to improve our knowledge and the quality of our services and we proudly achieved Practice Accreditation status and are also an Approved GP Training Practice. Our aim is to provide all patients who are seen and treated by the Doctors and Staff at Hawkhill with the highest standard of medical care and efficient administrative services from our hardworking Administration Team behind the scenes.
We would be extremely grateful if you would spend a little time going through our website. It has been put together to Help us to Help you.
One Click Away
Convenience doesn’t stop there and you can now access a number of services online. Ordering a repeat prescription is just a click away.
If you require any vaccinations relating to foreign travel you need to make an appointment with the Practice Nurse at least 6-8 weeks before the date of travel to discuss your travel arrangements.
We require a travel assessment form to be completed for each patient travelling. Please click here to download our travel assessment form. The form must be completed before any travel screening appointment can be made, please ensure you bring your completed form to your appointment.
We cannot provide advice on complex vaccination schedules, anti-malaria recommendations or travel prescriptions.
We can only provide the following travel vaccinations: Tetanus/Polio/Typhoid/Hepatitis A & Diptheria.
For further travel information, please click on Clinics & Services tab at top of home page.
Failure to Attend Appointments
Due to an increase in the number of wasted appointments (average 174/month) through patients failing to attend appointments without informing the surgery, it has become necessary to implement the following policy:
"If you fail to attend appointments you may be removed from this practice list and required to find an alternative doctor"
If you cannot attend your appointments for any reason please let us know as soon as possible, giving at best 24 hour notice. We can then offer the appointment to someone else.
Same Day Prescription Requests
Please allow sufficient time for the Practice Team to process your request. By law all prescriptions must be reviewed and appropriately prescribed. We cannot, therefore, guarantee that your request will be processed the same working day.
Please make every effort to request your prescription(s) at least 48-hours before they are required.
Our administration staff, and the Hawkhill website, can provide details about the ways in which prescriptions may be requested.
Thank you for your understanding.
