The Partners and Staff would like to welcome you to Hawkhill Medical Centre. The present Practice was established in 1993 when the GPs from three well established Practices in the West End of Dundee joined together to form a new partnership to consult from a modern purpose built medical centre.
Hawkhill Medical Centre was designed with their patients in mind with a vision to provide a wide range of healthcare services
We are always striving to improve our knowledge and the quality of our services and we proudly achieved Practice Accreditation status and are also an Approved GP Training Practice. Our aim is to provide all patients who are seen and treated by the Doctors and Staff at Hawkhill with the highest standard of medical care and efficient administrative services from our hardworking Administration Team behind the scenes.

NEW ONLINE PRESCRIPTION ORDERING SERVICE
As of 01.10.19 the current email and website online prescription ordering systems will no longer be available for you to order your medication.
The Practice is now offering a new service called "My Patient Services" where you can order your repeat medication more efficiently and also book appointments.
Please ask the Receptionist for a form to register for this service or alternatively click on this link to download the registration form: My Patient Services Registration Form
You will require to complete and return the form to the Practice before you can start using this service. Once we have received your form, we will send you by email your registration details which will guide you to creating your username and password.

Travel Advice Clinics
Please be advised that we no longer offer Travel Advice Clinics. We are still able to offer appointments for administering NHS vaccinations. For further information please see "Clinics & Services" tab at top of screen.
Patient Assessment and Liaison Mental Health Service (PALMS)
Are you worried about your mental health? Feeling overwhelmed or low and needing to talk to someone about seeking relevant support?
You can self refer to PALMS for consultation.
PALMS is a new service run by Dundee Health and Social Care Partnership within NHS Tayside now available in your GP practice.

CLOSED FOR TRAINING
The surgery will be closed on the following afternoons from 12:30 for training to allow practice staff to keep up-to-date with treatments, medication and clinical systems. If you require advice or treatment before we re-open, please contact your local pharmacy or NHS24 on 111.

Failure to Attend Appointments
Between 01.01.18 and 31.12.18 — a total of 1283 appointments were wasted due to patients booking appointments and then not attending.
We are an extremely busy Practice and you will agree that this is not acceptable and these appointments could be offered to other patients.
Please ensure that you cancel your appointment. You are at risk of being removed from the Practice List if you continually fail to attend your appointments.
The Practice operates a text reminder service which will send you appointment reminders and if you are unable to attend you can cancel by text up to one hour before your appointment. Please speak to our Admin Team if you wish to sign up to this service.
Same Day Prescription Requests
Please allow sufficient time for the Practice Team to process your request. By law all prescriptions must be reviewed and appropriately prescribed. We cannot, therefore, guarantee that your request will be processed the same working day.
Please make every effort to request your prescription(s) at least 48-hours before they are required.
Our administration staff, and the Hawkhill website, can provide details about the ways in which prescriptions may be requested.
